Job Summary/Purpose: Serves as the designated liaison/point-person for emergency departments, healthcare facilities, and community-based physicians/providers to coordinate a seamless and timely transfer to CHI Health tertiary hospitals. Work involves appropriate triage of requested needs for patients and implementation of defined protocols. Responsible for facilitating acceptance, bed assignment, transport and other care team notifications and documenting all actions. Also responsible for follow-up communication to customers and reviewing calls for quality assurance. Job duties require the services of a Registered Nurse who, based on training and work experience, uses professional knowledge and judgment in understanding and interpreting the clinical acuity/diagnoses of patients and integrating that data with real-time to ensure that patients are matched with the appropriate facility, level of care and unit. Key Job Responsibilities: Promote patient retention by providing focus on getting patients to the right service with the right provider at the right time - one call resolution. Recommends and contributes to the development of the Referral Center's services to fulfill patient needs and create a competitive advantage. Manage phone call activity to minimize any delays experienced by customers. Conduct intake for new referrals to quickly and accurately triage the needs of the caller. Document all actions related to calls using designated software program. Proactively monitor bed availability and communicate with House Supervisors and Unit Personnel as needed to understand issues and identify interventions to create capacity. Collaborate with team to handle phone call traffic and keep all requests moving. Coordinate transport services if requested. Review calls to identify adherence to protocols and opportunities for improvement. Participate in training of staff. Ensure that all activities comply with Healthcare Industry standards and government regulations including necessary accreditations and/or licensures. Minimum Qualifications: Required Education for Staff Job Levels: Bachelors of Science Nursing Required Licensure and Certifications: Registered Nurse Required Minimum Knowledge, Skills and Abilities: Knowledge of all applicable laws and regulations, i.e. EMTALA, CMS Conditions of Participation. Strong interpersonal communication and professional conduct with internal and external customers. Ability to provide positive customer service and effective verbal communication and active listening skills. Ability to solve practical problems and deal with a variety of concrete variables. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. Strong understanding of tertiary hospitals, services available, inpatient units/capacity and admitting practices and requirements. Strong understanding of regional referring hospitals, clinics and providers, particularly location in the region and services available. Computer proficiency with with MS Office Word, Internet navigation, database management, electronic medical record documentation and navigation. Ability to learn and utilize new technology. Proficient phone navigation skills. Preferred Qualifications: Minimum of five years clinical experience. Experience in call center environment.
