We are looking for a Phy sical Therapist to join our team at Heritage OnCare Home Health and Hospice!

Our Physical Therapists:

• Build positive relationships throughout the home health team and community we serve.
• Work closely with patients, families, and physicians to provide a personalized care experience by assessing individuals' abilities and needs.
• Seek out and learn new principles and methodologies of care to incorporate in clinical practices.
• Initiate and revise care plans to meet patient goals.
• Maintain records and ensure the care provided enhances the lives of our patients and meets state and federal guidelines.
• Ability to document efficiently, at the point of care, using a tablet. To include but not limited to OASIS assessments, recertification's, provider orders,
• Participate in interdisciplinary treatment team meetings.
  Our Physical Therapists Have:
  • A Bachelors or Masters degree in Physical Therapy and is licensed with the state of Nebraska.
  • At least 2 years of experience as a Physical Therapist with experience in Home Health or Hospice preferred.
  • A current Drivers license, acceptable driving record and access to reliable transportation.
  • The ability to independently lift 50lbs. Come Join Our Team
