Maple Crest Health Center, a part of American Baptist Homes of the Midwest, is looking for a Social Worker to join our team. This is a full-time day-shift position with rotating weekend on-call responsibilities. As a Social Worker at Maple Crest, you'll be responsible for providing assessment, counseling and supportive services in accordance with the interdisciplinary plan of care which promotes the mental, psychosocial, and spiritual health of residents along with the community's continuum of care. This includes conducting SS assessments and social histories, updating care plans, MDS's, and discharge planning. Will address resident and family concerns and handle abuse investigations. Refers residents and others to community resources and agencies as appropriate. What We Need From You:

• Degree in Human Services required. Bachelor's degree in social work from an accredited school of social work strongly preferred. Certified Social Worker certification (CSW) preferred.
• Social Work experience preferred; long-term care experience preferred.
• Must have knowledge of Social Security, Medicare and Medicaid.
• Ability to demonstrate positive attitude, emotional maturity to effectively communicate and interact with residents, families and co-workers.
• Requires strong critical thinking skills, independent judgment, and the ability to multi-task and prioritize work assignments.
