Responsibilities

CHI Health at Home is now hiring a Licensed Medical Social Worker to work within our Hospice division in Omaha, Nebraska

Job Duties listed:

• 
  
• Establishes a care plan in according to the physician's plan of treatment. Provides crisis intervention, as required.
  
• Reports changes in patient condition to appropriate personnel and the attending physician.
  
• Assists the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient's ability to improve medically.
  
• Provides patient care education to patient and other caregivers.
  
• Assists in making appropriate community referrals to gain access to additional resources, as needed.
  
• Works with the patient and caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
  
• Assists in making appropriate community referrals to gain access to additional resources as needed. (i.e. applications for public assistance, housing problems, additional health care resources, transportation).
  
• Participates in agency in-services, case conferences, discharge planning, summary reports, quarterlyrecord review and annual program review.
  
  
  Qualifications
  
  • 
    
  • Masters degree from an accredited school approved by the Council on Social Work Education.
    
  • Currently licensed/certified (if required) to practice in the state serviced.
    
  • One year social work experience in a health care setting required. Hospice experience preferred.
    
  • Valid Driver's License, car insurance & reliable transportation
    
  • Must obtain & maintain current CPR certification (American Heart Association or American Red Cross)
    
  • Intuitive knowledge of technology and comfortable with smartphones, tablets, virtual reality, interactive software and artificial intelligence
