individualized care plans for each patient Coordinate and provide
skilled nursing care to patients in their homes Collaborate with
other healthcare professionals to ensure the highest level of care
Educate patients and their families on their conditions and
treatment plans Provide emotional support and guidance to patients
and their families Qualifications: Valid RN license in the state of
Nebraska Prior experience in home health or case management Strong
communication and organizational skills Ability to work
