Nurse, where you will play a vital role in assessing, admitting,

with needs over the weekend Conduct in-home assessments to
determine eligibility for home health services Collaborate with
patients, families, and interdisciplinary teams to create

healthcare providers to ensure continuity of care Provide patient
education and support to promote self-management and independence
Ensure compliance with regulatory requirements and quality
standards Qualifications: Current RN licensure in the state of
Nebraska Previous experience in home health or hospice care
preferred Strong communication and interpersonal skills Ability to
