Nebraska. We design, develop, produce, sell, and support custom Healthcare IT

solutions. Our work focuses on the integration of real-time data streams in

support of providing care givers, patients, and family members access to

coordinated, holistic care information. We currently offer configurable

solutions for procedural area integration (e.g. operating rooms), fall

prevention, virtual care teams, and multi-source audio/video collaboration. In

addition, we offer scalable custom solutions for addressing room to enterprise

Monday-Friday 5:00 p.m.-12:00 a.m.

SUMMARY

The Licensed Practical Nurse (LPN) provides patient care

based on their state board of nursing "scope of practice". The LVN/LPN provides

nursing care to patients as part of the multidisciplinary team in accordance

with the hospital's mission, physician and patient/s expectations. The Licensed

Vocational Nurse/Licensed Practical Nurse shall be responsible and accountable

for making decisions based upon the individual's educational preparation and

experience in nursing.

PRIMARY RESPONSIBILITIES

All essential functions must be performed. Reasonable

1. Performs nursing

care for assigned patients within the limitations of licensure and nursing

department policy and licensure.


  • Observe patient/family needs in

    relation to presenting problem, care needs, physical condition and learning

    need.
  • Observation on all patients and

    follow up as per policy
  • Identifies problems and / or

    nursing diagnosis and plans for nursing interventions.
  • Anticipates patient care problems

    and initiates corrective actions.
  • Recognizes abnormal diagnostic

    laboratory data, blood gas values, radiologic data and reports results to RN or

    physician
  • Recognizes the patient's and

    family's perception of hospitalization and health status.
  • Identifies the patient's learning

    needs and implements appropriate measures to meet them including the family in

    the process.

    2. Implements

    nursing actions to achieve patient care goals.

    • Consistently uses tools available

      in the electronic medical record to ensure patient safety and optimize patient

      care. For example:



    • Patient Care management dashboard
    • Real time documentation.
    • Reviews and updates task list

      during the shift, including physician orders.
    • Performs bedside report using the

      EMR at each handoff report.
    • Establishes priorities of patient

      care based on the needs of the patient.
    • Implements a comprehensive patient

      care plan that includes baseline admission data, plans for treatment and goals

      for discharge.
    • Implements specific interventions,

      treatments, and medications as defined in the plan of care and documents

      outcomes.
    • Recognizes emergency situations and

      takes appropriate actions.
    • Complies with the National Patient

      Safety Standards regarding patient identification prior to all medications and

      treatments.
    • Accurately administers all

      medications in a safe manner displaying competence in the use of an automated

      dispensing machine where applicable. Educates patients and families regarding

      new medications including effects and side effects.
    • Maintains a safe environment for

      patients and families.

      3. Participate in

      the evaluation of quality of patient care and outcomes based on care and

      treatments administered.

      • Participates in multidisciplinary patient

        care conferences.
      • Confirms patient's understanding of

        care and treatments received.

        4. Assists the patient care team with

        formulating the plan of care and updates according to policy.

        5. Participates as a member of the Patient Care

        Team that includes: Virtual Care RN,

        bedside RN, and C.N.A.

        6. Accepts

        responsibility for one's own professional standards.

        • Accepts and adjusts to changes

          imposed by government regulations, consumer demands, professional

          organizations, and other external/internal forces.
        • Accepts responsibility for one's

          own actions by initiating the incident report mechanisms.
        • Maintains current clinical,

          technical, professional competencies
        • Attends a minimum of 75% of

          scheduled staff meetings annually.
        • Attends mandatory hospital

          in-services
        • Complies with departmental,

          divisional and hospital policies and standards.
        • Copes with emergencies, staffing

          shortages, shift rotations and floating assignments.
        • Demonstrates awareness of the

          rising costs of health care and implements budgetary containment measures.
        • Maintains equipment in its proper

          working condition, identifies malfunctioning equipment and takes appropriate

          action.
        • Reviews current literature and

          available
        • Seek and accept constructive

          criticism and implement actions designed for self-improvement
        • Show's self-direction and

          leadership by functioning without the need for close supervision
        • Other duties as assigned

            EDUCATION

            Education: Graduate

            of an accredited school of nursing with a Licensed Practical Nurse (LPN)

            required.

            Experience: 3 years clinical experience in focus area

            preferred.

            License/Certifications: Current Nebraska Licensed Practice

            Nurse license required. Current Licensed Practical Nurse license required in

            state where the patient is located will be required immediately upon hire. Experience in a supervisory or quality assurance position in

            acute and/or home health, preferred.

            Special qualifications: Manage and communicate data in a

            nursing informatics work environment to Managed Services Clients, nurses,

            health care providers and patients. Act as the liaison between nurses and

            technology people. Must be able to successfully complete established

            competencies for the position within designated probationary period. Effective communication skills; both orally

            and written. Computer skills

            needed. Ability to work with others

            within a team to ensure quality patient care.

            Strong critical problem-solving skills. Ability to respond to common

            inquiries or complaints from customers or regulatory agencies. Ability to

            define problems, collect data, establish facts and draw valid conclusions.

            Ability to work independently and under pressure.

            Hours of work: PRN,

            Part-Time and/or Full-Time Non-Exempt - 12-hour shift, rotating weekends.

            • Experience and knowledge of the

              following Microsoft System: Active Directory, Exchange,
