Looking for a LPN who wants to join at strong stable organization and be able to grow with us! CHI Health at Home is looking for a full-time Nurse Support Coordinator I to join our Home Care team! OVERVIEW The Nurse Support Coordinator supports the location's top Director and team members in the completion of daily activities to optimize the clinical staff's time in caring for clients. Facilitates the completion of documentation to comply with regulatory, operational and financial requirements. QUALIFICATIONS AND COMPETENCIES 1. Current LPN license in good standing to operate in state(s) is required. 2. Strong clinical skills. 3. Two years related experience. 4. Excellent organizational, interpersonal and communication skills. 5. Good computer skills, familiar with multiple software programs including Windows based programs. DUTIES AND RESPONSIBILITIES General 1. Supports CHI Health at Home's mission, values, strategic goals and high standards of customer service. Consistently lives People First Behaviors. 2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information. Maintains confidentiality regarding all patient, personnel, and organization information. 3. Works independently with limited direct supervision using the guidelines CHI Health at Home and branch policy and procedures and good judgment. 4. All other duties as assigned. Clinical Support 1. Acts as a clinical intermediary for the field staff including but not limited to: a. Updates or changes in physician orders b. Takes laboratory results and communicates results appropriately c. Appropriately enters ICD-9 codes 2. Ensures the processes for physician orders (485's), recerts, OASIS assessment data entry, updates and changes are completed accurately and in a timely manner. 3. Acts as a liaison with corporate Revenue Cycle Department to resolve questions and issues provide necessary documentation, etc. 4. Takes referrals and completes the intake according to established processes ensuring complete and accurate information. 5. Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services. Notifies Director as needed to ensure necessary information/documentation obtained. 6. Audits charts as requested. 7. Completes clinical documentation, (i.e. discharge summaries, etc.) according to company policy. 8. Communicates with additional service providers, (i.e. HME, supplies vendor, etc.) as needed. 9. Assists in training new personnel as requested by management. Assists in identifying clinical training needs. 10. Assists the Director in review of processes and paperwork as requested. Completes required branch and/or company reports as required. 11. Acts as a resource for external customers, including but not limited to, physicians, referral sources and discharge planners/social workers. 12. Facilitates the resolution of client clinical issues. 13. Provides clinical expertise for internal customers. 14. Supports coordination of care through case conferences and other activities for interdisciplinary care. 15. Performs client visits as needed and assigned by management. Office Support 16. Performs office functions as needed according to the number of positions covering all office functions and may fill a hybrid position comprised of "Nurse Support Coordinator II" and "Office Support Coordinator" position responsibilities, 17. The additional office responsibilities are detailed in the "Office Support Coordinator" job description. The Office Support Coordinator performs a wide range of functions critical to the success of the organization's branch location: a. Support - customer and clinical b. Intake c. Health information - data entry and medical records d. Employment - talent acquisition, on-boarding, scheduling and payroll 18. The hybrid position serves as the primary point of contact and coordinator of activities for all external and internal customers, including patients, other health care providers and staff.
