6:00 am - 2:30 pm
Provides knowledge and expertise in delivering prescribed exercise
programs for residents. Suggests additional programs and provides
information related to exercise/restorative programs. Assists in
restorative care of residents as
directed.Responsibilities:Essential Functions Exhibits excellent
interpersonal relations with patients, co-workers, physicians and
families to ensure effective communication.
- Communicates with other members of the health care team
consistently and appropriately.
- Responds to resident needs promptly and courteously.
- Communicates resident issues to appropriate staff.
- Maintains a positive attitude while responding to requests in a
timely and accurate manner. Completes clinical activities
identified in the residents plan of care to meet
comfort/activity/rest/nutrition needs and other clinical needs as
delegated by professional staff.
- Accurately carries out the therapeutic exercise program and
ambulation as defined in treatment plan.
- Provides assistance with repositioning, turning and chair
transfers using correct body mechanics.
- Consistently reinforces resident education regarding
therapeutic exercise and ambulation, etc.
- Consistently reports resident comfort needs to nurse and/or
therapist in a timely manner.
- Consistently monitors skin condition and reports any skin
changes to nursing staff.
- Accurately assists with vital signs/weights/personal
care/Activities of Daily Living (ADL's) as needed.
- Accurately records intake/output.
- Safely provides transportation assistance as needed. Documents
all data collected, in EMR and/or worksheets, accurately,
completely and in a timely manner to provide an accurate record of
patient activities and response to activities.
- Documentation is consistently and accurately completed prior to
end of shift.
- Documentation consistently and accurately reflects residents
responses to activities according to plan of care.
- Documentation consistently and accurately reflects care given.
Consistently recognizes and utilizes measures to provide for safe,
efficient, resident care and work environment.
- Responds to resident requests and /or relays request to
appropriate resource in a timely manner.
- Willingly assists other team members when own work is
complete.
- Appropriately reports all incidences.
- Utilizes Universal Precautions correctly and consistently.
Assists in the care of the resident by performing delegated
activities utilizing the reciprocal process of the 4 Rights of
Delegation.
- Right task - within scope of practice, according to the job
description, and based on the desired outcome, task routine,
non-complex, low risk and predictable outcome.
- Right person - assess competency by certification/licensure,
job description, skills checklist, demonstrated skill.
- Right communication - clear, concise, correct, complete.

Description:Job RequirementsEducation
- High school diploma, General Educational Development (GED) or
equivalent required.License/Certifications
- Current American Heart Association Basic Life Support (BLS)
required.
- Active Medication Aide Registry (MAR) required.Experience
- Requires skills in general office duties and basic medical
terminology.
- Requires the ability to learn computer operations, therapy
equipment.Skills
- Requires skills in communication both oral and written, time
management and organization, and personnel management.
- Requires the ability to work independently, adapt to a changing
